Trusts and estates (Forms 1041 and 706)

  • Death certificate
  • Signed will or trust agreement
  • Letters testamentary or court appointment
  • EIN for the trust or estate
  • Social Security numbers, addresses, and contact information for beneficiaries and fiduciaries
  • Prior year's return
  • List of trust or estate assets, including valuations
  • Income documents for the trust or estate and for the decedent, including K-1s
  • Deductions and expenses
  • Distributions paid to beneficiaries during the year
  • Charitable contributions

Gifts (Form 709)

  • A list of every gift made during the calendar year, with the recipient's name, relationship to you, date of transfer, and a description
  • Whether your spouse agrees to split gifts (your spouse's signature is required)
  • Qualified appraisal reports for gifts of real estate or business interests
  • Brokerage statements for gifts of stock
  • Records showing your original cost basis in the gifted property
